Consistent Coffee Consumption Linked to Reduced Risk of Serious Liver Disease

A new study has found that regular coffee consumption, including both caffeinated and decaffeinated varieties, is associated with a lower likelihood of developing serious liver ailments. According to researchers at Cedars-Sinai, individuals who drink coffee on a daily basis show fewer cases of cirrhosis, liver cancer, and liver-related death compared to those who do not consume the beverage.

Study Methodology and Findings

The research, published in Clinical Gastroenterology and Hepatology, involved an analysis of 354,957 adults from the UK Biobank who did not have liver cancer or cirrhosis at the start of the study. Investigators followed these participants for a median of 13 years, utilizing linked health records to track the development of liver disease. To understand the biological mechanisms behind these outcomes, the team went beyond simple dietary questionnaires. They combined more than a decade of health records with liver MRI scans and blood protein analyses. The findings revealed that participants who consumed higher amounts of coffee tended to exhibit lower levels of liver fat, iron, fibrosis, and inflammation on their MRI scans. Furthermore, blood protein data showed that coffee drinkers had higher levels of proteins associated with healthy liver function and lower levels of those connected to scarring and inflammation. The Role of Non-Caffeine Compounds

One significant finding of the study is that the observed benefits were similar for both caffeinated and decaffeinated coffee. This suggests that caffeine is likely not the sole driver of these liver-protective associations. Scientists believe that other naturally occurring chemical compounds in coffee may influence pathways related to inflammation, scarring, and liver metabolism. Specifically, chlorogenic acid has been identified as a compound that may improve how effectively the liver responds to insulin.

Practical Implications and Moderate Intake

While the study identified the lowest liver health risks among those who consumed five or more cups daily, investigators do not recommend that individuals increase their intake to that level. Researchers noted that benefits were observable at one to two cups per day and appeared strongest at three to four cups. “Our findings support moderate coffee consumption for people who already enjoy and tolerate it well,” said Dr. Ju Dong Yang, the study’s senior author and medical director of the Liver Cancer Program at Cedars-Sinai. “However, we would not recommend that someone begin drinking coffee solely for liver protection based on this study alone.” The study authors emphasized that coffee consumption should not replace established medical care or healthy lifestyle habits. Prevention of liver disease remains focused on maintaining a healthy weight, exercising regularly, limiting alcohol consumption, and managing blood sugar, blood pressure, and cholesterol.

Context and Limitations

Because the study was observational, it demonstrates an association rather than proving that coffee directly prevents liver disease. The research team aims to continue their work by moving from broad dietary patterns to identifying specific compounds in coffee that may be responsible for these health benefits. “Our findings point to biological pathways involving inflammation and scarring and highlight molecular targets that future research can explore to better understand how coffee may influence liver health and who stands to benefit the most,” said Dr. Shelly Lu, director of the Karsh Division of Gastroenterology and Hepatology at Cedars-Sinai.
