High-Dose Vitamin D Linked to Better Cognition in High-Risk Adults

Adults taking high doses of vitamin D supplements showed significantly higher cognitive test scores in a new study of high-risk patients published in Sleep Medicine. The findings emerge alongside large-scale international research linking vitamin D intake to lower dementia risks, though health experts warn high supplement doses carry documented medical risks.

With millions of Americans living with Alzheimer’s dementia—a projection expected to reach nearly 13 million by 2050—new Emory University research suggests that higher vitamin D supplement intake is tied to better cognitive function in adults facing an elevated risk of the condition. Researchers focused on a group of 54 adults experiencing sleep disturbances and mild cognitive impairment, which serve as early hallmarks of dementia.

Participants in that study who reported taking at least 5,000 international units of vitamin D daily scored more than 13% higher on the Montreal Cognitive Assessment compared to individuals who did not take high-dose supplements. Lower daily doses showed no such association with higher cognitive test scores.

Senior study author Victoria Pak emphasized that the timing of such interventions during early cognitive decline is critical. According to the Emory study, researchers also noted that cognitive performance did not differ whether participants took vitamin D2 from plants and fungi or D3 produced via sunlight and animal-based foods.

Large-Scale Population Data and Brain Protein Accumulation

In a separate midlife investigation published in Neurology Open Access, researchers measured vitamin D blood levels in roughly 800 adults averaging 39 years old. About 16 years later, PET brain scans revealed that participants with higher vitamin D levels in early adulthood accumulated lower levels of tau protein in their fifties.

Clinical Warnings and Dietary Sources Versus Supplements

Despite promising observational data, health authorities and clinical specialists urge caution regarding high-dose regimens. High doses can trigger hypercalcemia, a dangerous buildup of blood calcium that deposits in soft tissues and arteries, alongside increased risks of painful kidney stones.

Furthermore, major clinical trials present a mixed picture. The nationwide Vitamin D and Omega-3 Trial found no reduction in heart attacks, stroke, cancer, or depression among participants taking supplements, though long-term users showed lower cancer mortality rates and a reduced risk of autoimmune diagnoses.

Ongoing Clinical Trials and the Path Forward

Ongoing efforts such as the VitaMIND study run through the PROTECT platform at the University of Exeter are actively testing vitamin D versus placebos over time.
